  To help determine your initial suitability for egg donation, please take a few minutes to review the list of preliminary qualification statements. If you meet the basic criteria, we invite you to take the next step in becoming an egg donor with our program by completing our online egg donor application form. Please read and confirm all of the following initial qualification statements before continuing.
  As a potential egg donor candidate, I acknowledge that:
- I am between the ages of 19-30 years old.*
- I am looking to help someone become a parent through the donation of my eggs.*
- I am mature, responsible, reliable, and dependable.*
- I am physically and emotionally healthy.*
- I am free of known genetic diseases, and there are no major genetic diseases in my immediate family (parents, siblings).*
- I am able to gather accurate and up-to-date health information regarding my immediate and extended family members.*
- I have normal menstrual periods.*
- In the past two years, I have not used Depo-Provera, Norplant or Implanon as my method of pregnancy prevention/birth control.*
- I am a non-smoker/non-tobacco user.*
- I will not use recreational drugs, including marijuana, even if marijuana is legal for recreational use in my state of residence.*
- I am not currently taking (nor have I taken in the past) any form of anti-depressants or anti-anxiety medication.*
- I have not been diagnosed with, or otherwise treated for, any sexually transmitted disease(s) in the prior 12 months.*
- I am willing to undergo comprehensive medical screening, including but not limited to, a physical exam, communicable disease testing, genetic testing, psychological testing, and drug screening.*
- My body mass index (BMI) falls within the recommended guidelines of 18-28.*See BMI Chart
- I am willing to administer 10-12 days of injectable medication.*
